4. Receiving Bank Form: To receive a lottery prize amount, you are required to submit this Receiving Bank Form and that will allow Kerala State Lotteries Departmenrt to directly credit prize money to your account.

6. Nominee Form (Agent): You can download the agent nominee form using the below, which will help to collect the money while the agent is not available.

7. Agency Prize Claim Form: When you sell a prize-winning lottery ticket, you as an agency, are entitled to receive a commission of 10-12% of the winning amount. You're required to submit the agency prize form to receive the commission.
